Chronic Kidney Disease: Diagnosis and Treatment

Nephrology  Comments Off on Chronic Kidney Disease: Diagnosis and Treatment
Nov 142019

This book provides a comprehensive and systematic review of the latest findings in a wide spectrum of clinically important aspects of chronic kidney disease (CKD), focusing on clinical diagnosis and therapeutics. CKD is a global health problem with a rising morbidity and mortality. The last decade has seen significant improvements in determining the incidence, prevalence, and complications of CKD, mainly thanks to the definitions of CKD developed by the National Kidney Foundation’s Kidney Disease Outcomes Quality Initiative (K/DOQI). However, increased recognition of CKD has led to awareness of the limitations of its clinical diagnosis and treatment, which are essential to patients’ wellbeing. This book is of particular value not only to nephrologists, but also to general practitioners and residents with an interest in CKD. It offers a well-organized exposition of the current knowledge base. Compared with previously published books on kidney disease and CKD, it has a smaller number of more concise chapters. As a result, readers can easily obtain an overview of the most important topics in CKD. We hope that practitioners will gain as much from reading this practical guide to clinical management of CKD as we have from editing it.

Informing Clinical Practice in Nephrology: The Role of RCTs

Nephrology  Comments Off on Informing Clinical Practice in Nephrology: The Role of RCTs
Oct 312019

Historically, clinical decisions in renal medicine have been challenged by the scarce availability of robust supportive evidence. Not only are the number of randomized controlled trials (RCTs) in Nephrology the third lowest amongst the medical specialties but in many instances the trials themselves are of poor quality. In addition, practice has been further influenced by extrapolation from the outcomes of general population clinical trials which exclude renal patients. The difference between the ideal trial participant and real complex cases encountered in daily practice is well recognized and further compounded in renal patients with complex pathophysiology – this ultimately makes decision making in this subset of patients a real challenge. Recently, there has been a growing interest in conducting well designed RCTs in different areas of renal medicine. However, though clinical guidelines are helpful in providing the clinicians with a frame of best available evidence for a clinical condition, it denies the unique nature of each individual patient.

This book offers a thorough and critical appraisal and evaluation of the key published clinical trials that have shaped current practice in nephrology, dialysis and transplantation. It will help the practicing physician close the gap between the inflexible and generalized nature of clinical guidelines and the day-to-day clinical decision-making for individual patients. It will provide the clinician with the tools required to investigate and extract the appropriate guidance to apply to individual cases in daily practice. Moreover, it will help improve the ability of junior colleagues to appraise available evidence in a systematic way when there is lack of local guidelines or when the guidelines are difficult to apply due to logistic constraints or barriers. Lastly, this book will serve as a reference for key clinical trials in different areas of renal medicine together with literature and authors views of these trials and their impact on changing practice.

The Doctor’s Kidney Diets: A Nutritional Guide to Managing and Slowing the Progression of Chronic Kidney Disease

Nephrology  Comments Off on The Doctor’s Kidney Diets: A Nutritional Guide to Managing and Slowing the Progression of Chronic Kidney Disease
Oct 222019

* Winner of the IBPA Benjamin Franklin Award for Best Health Title

In the United States alone, 26 million adults have chronic kidney disease (CKD), and experts project that over half the country may develop CKD due to rising rates of disorders such as diabetes. While nephrologists can monitor kidney function and treat patients with medications, they can’t always offer the nutritional guidance that every kidney patient requires. To fill this information gap, Dr. Mandip Kang has written The Doctor’s Kidney Diets, a comprehensive guide to managing, slowing down, and even stopping the progression of CKD through diet.

The book is divided into two parts. Part One provides a clear overview of kidney function, kidney disease, and the role that nutrition plays in the treatment of kidney problems. The doctor then reviews the special dietary considerations of individuals with CKD, including the need to limit certain nutrients, fluids, and other dietary components. Because different patients have different nutritional requirements, the doctor discusses the most commonly prescribed CKD diets―the DASH diet, heart disease and diabetes diets, diets for dialysis, and more―and concludes with important tips for enhancing overall health and maximizing treatment success. Then Part Two offers a wide variety of recipes for dishes that follow the dietary guidelines highlighted in Part One.

Smart nutrition is essential to the treatment of kidney disease. With The Doctor’s Kidney Diets, you can become an active, effective participant in your own treatment plan.

Doody’s Review Service Review by Sumit Narula, MD (University of Kentucky College of Medicine)

This is the fifth edition of a concise, yet comprehensive review of all the modalities of dialysis and associated complications and clinical management. The ebook version that accompanies the book is user friendly and covers most of the information in the book. The goal is to provide a review of the commonly used dialysis modalities and management of associated issues. The book begins with a chapter on evaluation of predialysis patients, followed by thorough discussions of dialysis physiology, various dialysis modalities, and dialysis catheter complications. Finally, it addresses common psychosocial and medical conditions afflicting dialysis patients.

This will be an excellent tool for residents, renal fellows, general nephrologists, physician assistants, nurse practitioners, and internists. The book provides concise yet thorough information on the management of dialysis patients based on current evidence and practice guidelines. It also covers dialysis management in children and during pregnancy and includes a chapter on the management of bone disease. The 40 chapters are divided into four parts. The first part discusses management of advanced CKD patients and their transition into dialysis, renal transplant, or palliative care. The next 18 chapters discuss the basics of hemodialysis and dialysis access, followed by complications. The next part has extensive discussions of peritoneal dialysis, while the final part addresses the medical conditions and psychosocial issues affecting dialysis patients. Each chapter provides a concise though complete description of dialysis with its associated condition with tables and pathological figures where appropriate. All chapters include references and suggested readings. As the name suggests, the handbook delivers precise information on all aspects of dialysis. Simple tables and figures are used appropriately and increase the ease of reading. Although this book is well known in the nephrology community, it will be a useful tool for nurse practitioners and physician assistants for understanding the core concepts of the various modalities of dialysis. Care of dialysis patients requires a multifaceted approach and the handbook serves its purpose by covering all the important aspects that concern dialysis patients. The attempt to compare delivery of dialysis internationally also broadens the knowledge base and deepens understanding of dialysis patient management. I would recommend this book to anyone who aspires to completely grasp the basic as well as advanced concepts for management of dialysis patients.
