Hysterectomy: Procedures, Complications and Alternatives

Ob/Gynae, Surgery  Comments Off on Hysterectomy: Procedures, Complications and Alternatives
Feb 272018

Hysterectomy is one of the most prevalent surgeries world-wide in the female population. Nine out of every ten hysterectomies are performed for non-malignant conditions that are not life-threatening, but have a negative impact on quality of life. Indication policy must be revised as new treatments become available.

Menorrhagia is the primary indication and it is not always a response to an anatomical disease; other common indications are genital prolapse and malignancy. New and improved alternatives are increasingly employed for this indication and are responsible for the fall in the rate of hysterectomies performed in the last decade. This book presents current research in the study of hysterectomies, including laparoscopic hysterectomy; beliefs, attitudes and psychological reactions to hysterectomy; myomectomy in place of hysterectomy; surgical management of congenital uterine abnormalities; and, the hysterectomy crossroads.

Abortion and the Private Practice of Medicine

Ob/Gynae  Comments Off on Abortion and the Private Practice of Medicine
Feb 192018

Originally published in 1986, Abortion and the Private Practice of Medicine was the first book to look at abortion from the perspective of physicians in private practice. Jonathan B. Imber spent two years observing and interviewing all twenty-six of the obstetrician-gynecologists in “Daleton,” a city that did not have an abortion clinic. The decision as to whether, when, and how to perform abortions was therefore essentially up to the individual doctor. Imber begins the volume with a historical survey of medical views on abortion and the medical profession’s response to the legalization of abortion in the United States. Quoting extensively from his interviews, he looks at various characteristics of doctors that may affect their professional opinion on abortion: their age, gender, religious background, and length of residence in the community; the nature of their training and prior experience; and the setting of the practice (whether group or solo). Imber found that the physicians’ reasons for agreeing or refusing to perform abortions revealed considerable differences of opinion about how they construe their responsibilities.

Infertility in Women with Polycystic Ovary Syndrome: Pathogenesis and Management

Ob/Gynae  Comments Off on Infertility in Women with Polycystic Ovary Syndrome: Pathogenesis and Management
Feb 132018

This book presents up-to-date knowledge on infertility in the context of polycystic ovary syndrome (PCOS) and provides clear evidence-based guidance on its treatment. The book opens by discussing anovulation, oocyte quality, and the endometrium in women with PCOS, infertility and subfertility cofactors, and the impact of PCOS phenotypes on fertility. All aspects of management are then thoroughly addressed.
The available medical treatments for PCOS-related infertility – including antiestrogens, aromatase inhibitors, insulin-sensitizing drugs, and gonadotropins – are reviewed, and other potential therapeutic approaches, such as acupuncture and laparoscopic ovarian drilling, are assessed. Careful attention is also devoted to the role of lifestyle interventions. The use of controlled ovarian stimulation in infertile PCOS patients undergoing intrauterine insemination or in vitro fertilization is examined in detail, as are the benefits of in vitro maturation of oocytes. This book will be of value to all who are involved in the care of women with PCOS and related infertility issues.
