A growing body of research indicates that social determinants of health have a significant impact on health care utilization and outcomes. Researchers and policymakers in the United States have spent decades exploring and discussing approaches to integrating health care and social services. While no nation has a truly integrated system, many other industrialized nations invest more heavily in social services than the United States, and are more effective in integrating these services with health care. Integrating health care and social services, such as accessible housing, meals and nutrition services, transportation, and caregiver training, is particularly important for those facing serious illness who typically encounter multiple chronic conditions, pain and other symptoms, functional dependency, frailty, and significant family caregiver needs.

In an effort to better understand and facilitate discussions about the challenges and opportunities related to integrating health care and social services for people with serious illness, the National Academies of Sciences, Engineering, and Medicine held a full-day public workshop on July 19, 2018 in Washington, DC. The workshop featured a broad range of experts and stakeholders including researchers, policy analysts, patient and family caregiving advocates, and representatives of federal agencies. This publication summarizes the presentations and discussions from the workshop

The question of how and by what standard an individual should be declared dead is once more a matter of controversy. With this book, the President’s Council on Bioethics takes up this controversy and seeks to illuminate the issues at the centre of the renewed debate about the inherently perplexing problems of determining human death in an age of life-sustaining technologies. In most human deaths, the loss of neurological function is accompanied by the traditional, familiar markers of death – the patient stops breathing, his or her heart stops beating, and the body starts to decay. In relatively rare cases, however, the irreversible loss of brain-dependent functions occurs while the body, with technological assistance, continues to circulate blood and to show other signs of life. In such cases, there is controversy and confusion about whether death has actually occurred. In this book, the President’s Council examines the main lines of criticism and defence of the neurological standard, and also explores the ethical concerns engendered by the use of the traditional cardiopulmonary standard in the organ procurement practice known as ‘controlled donation after cardiac death’. In doing so, the President’s Council on Bioethics aims to appraise the American public of the contemporary state of the debate and to guide the publics reflections on matters that touch on some of society’s deepest human questions. Human Oral Mucosa: Development, Structure and Function is a new text that reflects the considerable increase in knowledge of oral mucosa that has occurred in recent years. Our understanding of the structure of oral mucosa is now established at a molecular rather than a tissue or cellular level. This in turn has revealed a level of function that was previously not suspected, including a sophisticated barrier to the penetration of exogenous materials, and the synthesis of specific antimicrobial compounds, representing components of the innate immune system.
