This book discusses medicine from an ethical perspective, whereas books on medical ethics more commonly present ethics from a bio-medical standpoint. The book is divided into 23 chapters. The introductory chapters present some basic concepts of medical ethics, such as the relation between the legal system and ethics, ethical documents, ethical theories, and ethical analysis. The following chapters address issues of importance in all fields of medicine: respecting autonomy, communication, relations within a healthcare team, professional malpractice, limited resources, and the portrait of a physician. In turn, the third part of the book focuses on ethical aspects in a broad range of medical activities – preventive medicine, human reproduction, genetics, pediatrics, intensive care, palliative medicine, clinical research, unproven methods in diagnostics and treatment, and the role of physicians who aren’t directly responsible for patient care. The last part presents students’ seminars with case stories. Andrew Thompson, Norman J. Temple We humans are an extraordinary species. One of our finest achievements is the development of morality, of a sense of right and wrong. We articulate and then impose this sensitivity upon ourselves in the form of ethical guidelines, rules, regulations, and laws. We have, regrettably, also developed marvelously clever ways of justifying our behavior whenever it runs afoul of these prescriptions. We have, for example, developed the concept of objectivity to guide scientific pursuits and subsequently established rights which undermine the possibility of ever coming close to attaining the goal of being objective — rights which entitle participating scientists to gain personal, tangible profits from scientific discoveries. Formerly, we envisaged gods who kept us in place, who reminded us that we were not all-powerful or especially wise. Now we tend to worship our achievements, especially our technological ones, and ourselves. Mary Midgley’ aptly names this phenomenon, “humanolatry.” We have lost our respect for nature in our enthusiasm for changing it to that which suits our shortsighted ends. We must, as she says, “unlearn” this way of thinking.

Rather, it presents the process of self-change from several different perspectives – environmental, cross-cultural, prevention and interventions at both societal and individual level. It provides strategies for how health care practitioners and government policy makers alike can aid and foster self-change. Directions for future research priorities are also presented.

As the fields of philosophy of medicine and bioethics have developed in the United States, the philosophical perspective of phenomenology has been largely ignored. Yet, the central conviction that informs this volume is that phenomenology provides extraordinary insights into many of the issues that are directly addressed within the world of medicine. Such issues include: the nature of medicine itself; the distinction between immediate experience and scientific conceptualization; the nature of the body – and the implications of embodiment in the realm of clinical practice; the meaning of health, illness and disease; the problem of intersubjectivity – particularly with respect to achieving successful communication with another; the complexity of decision-making in the clinical context (and in the realm of medical ethics); the possibility of empathic understanding; the theory and method of clinical practice; and the essential characteristics of the therapeutic relationship – i.e. the relationship between the sick person and the one who professes to help. At a time when it is becoming usual for medicines to be developed for a global market and pharmaceutical companies are endeavouring to expedite the drug development process, Regulatory Authorities are concentrating on improving their efficiency and effectiveness. Therefore, it is not surprising that questions are being asked as to how performance might be measured and compared between different authorities who are now often in receipt of dossiers that have been submitted to several agencies at the same time. Issues such as ‘what target should be set for the review of new medicines?’ and `how can quality be assured?’ are now considered to be of critical importance. The twelfth CMR International Workshop, held in January 1997, provided the opportunity for Regulatory Authority and industry personnel from Europe, North America, Australia and Japan to openly discuss experiences and exchange views on how to improve the review process. The proceedings of this meeting provide a comprehensive overview of the current review process in different countries and the need for performance measures and targets. This volume summarises the many suggestions that were debated at the Workshop, and includes chapters on measuring performance, and on the integration of quality into the review process.

About the Author
Susan Rvachew, PhD, S-LP(C), worked as a speech-language pathologist in pediatric health care settings for 20 years before taking a position at McGill University, where she is currently an Associate Professor in the School of Communication Sciences and Disorders. Her research is focused on phonological development and disorders and the development of more effective interventions to treat phonological disorders in children and prevent reading disability in this population. She has published over 50 journal articles and book chapters describing the speech perception, speech production, and/or phonological awareness skills of infants, children, and adults.

Françoise Brosseau-Lapré, MSc(A), SLP©, has worked as a speech-language pathologist in pediatric health care settings since obtaining a master’s degree in speech-language pathology in 2002 from McGill University. She is currently a doctoral candidate at the School of Communication Sciences and Disorders at McGill University. Her research focuses on French phonological development and disorders, as well as efficacy of interventions aimed at improving the phonological skills of preschool francophone children with developmental phonological disorders
